What this site collects, how we use it, and your choices

Effective October 10, 2026

Simi Valley Emergency Physicians ("SVEP", "we", "us") operates this website at www.svep.med. This policy explains what information the site collects, how we use it, and the choices you have.

SVEP is the physician group that staffs the emergency department at Adventist Health Simi Valley. This website is SVEP's own site. It is separate from the hospital's systems, patient portal, and medical records, and nothing you enter here becomes part of a medical record.

1. This site is not for medical information

We do not collect health information through this website, and we ask you not to submit any. Do not include diagnoses, symptoms, medications, dates of birth, medical record numbers, billing account numbers, or details of treatment in any form on this site.

For questions about your care, your medical records, or a bill, contact Adventist Health Simi Valley directly. In an emergency, call 911 or go to the nearest emergency department.

2. Information we collect

Contact form. Your name, email address, and message. These are emailed to our team so we can reply.

Patient and family feedback. Your message, and if you choose to give them, your name, contact details, and the name of the provider you saw. You confirm before sending that the message contains no medical information. Feedback is emailed to the SVEP physicians who review it.

Job applications. The position you apply for, your name, email address, phone number, current employer, years of experience, license state and number, board certifications, cover letter, how you heard about us, and the CV you upload. Applications are stored in our systems and shared with the SVEP physicians involved in recruiting.

Member portal (SVEP physicians, advanced practice providers, and staff). Sign-in uses your SVEP Google Workspace account. The portal stores your profile (name, work email, phone, alternate email, NPI, and facility), the content you post (updates, comments, discussion topics, and files), read receipts and notifications, and the questions you ask the knowledge-base assistant.

Technical data. Like most websites, our hosting provider records server logs that include your IP address, browser type, and the pages requested. We use these for security and troubleshooting. We do not run advertising trackers or analytics cookies.

3. How we use information

  • To answer your message or feedback.
  • To evaluate job applications and contact applicants.
  • To run the member portal: sign-in, group communications, shared documents, and the knowledge-base assistant.
  • To keep the site secure and to diagnose problems.

4. Service providers

We use a small number of companies to run the site. Each one processes information only to provide its service to us.

  • Vercel hosts the website and keeps its server logs.
  • Supabase provides the database, sign-in, and file storage, including uploaded CVs and portal files.
  • Resend delivers the emails the site sends, such as contact messages, feedback, and portal notifications.
  • Google provides Workspace sign-in for the member portal.
  • OpenAI processes the knowledge-base assistant in the member portal: staff questions and the text of SVEP's internal reference documents. No patient information is sent to it.

We do not sell personal information and we do not share it for advertising.

5. Cookies

The public site works without cookies. The member portal sets the essential cookies needed to keep you signed in and to remember your preferences. Details are in our Cookie Policy.

6. How long we keep information

  • Contact messages and feedback: as long as needed to respond and follow up.
  • Job applications and CVs: for the recruiting cycle they relate to, or until you ask us to delete them.
  • Member portal data: for as long as you are affiliated with SVEP, after which access is removed. Group communications may be retained as part of SVEP's business records.

7. Security

The site is served over HTTPS. The member portal is limited to authorized SVEP accounts, with access controls that restrict what each role can see. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.

8. Your choices

You can ask us what information we hold about you, ask us to correct it, or ask us to delete it, by emailing admin@svep.med. We will confirm your identity before acting on a request and will respond within a reasonable time. California residents have these rights under California law and will not be treated differently for exercising them.

9. Children

This site is not directed to children under 18, and we do not knowingly collect information from them. A parent or guardian may use the feedback form on a child's behalf, subject to the no-medical-information rule above.

10. Changes to this policy

We may update this policy as the site changes. The effective date at the top shows the current version. Material changes will be posted on this page.

11. Contact

Questions about this policy can be sent to admin@svep.med or through the contact page.
